Immunology

The science of the body's defenses — and the discovery that immunity is a physical substance carried in the blood, one that can be drawn from a recovered animal and given to a sick one to neutralize a poison or a germ.

In 1890 Emil von Behring and Kitasato Shibasaburō showed that the blood serum of an animal that had survived tetanus or diphtheria could be injected into another animal and protect it — immunity was a transferable thing in the liquid of the blood. Within a few years, the antitoxin that saved children from diphtheria was being brewed, by the bucketful, in the veins of immunized horses.

Immunology is the study of how a body distinguishes itself from what invades it, and destroys the invader. Its founding discovery was concrete and startling: when an animal survives a disease, its blood comes to contain a specific defensive substance — von Behring and Kitasato called it an *antitoxin*, and we now call the class of them *antibodies* — that latches onto the toxin or germ and disarms it. Crucially, that substance is not welded to the animal. Spin the cells out of its blood and the pale remaining liquid, the serum, still carries the protection. You can move immunity from one body to another in a syringe.

This turned defense into a thing you could harvest, store, and administer. It gave medicine its first reliable cure for diphtheria — a disease that had strangled children by the tens of thousands, choking them with a membrane across the throat — by taking serum from a large immunized animal and using it to neutralize the toxin already in a patient. It split immunity into two kinds that still organize the field: the slow, lasting kind you build yourself (as a vaccine trains you), and the instant, borrowed kind you can be handed in an antitoxin. And it launched a century of questions — what *are* antibodies, how does one body make millions of different ones, why does the system sometimes attack the self — that became modern immunology.

Working in Robert Koch's institute in Berlin, von Behring and Kitasato injected animals with steadily larger, sub-lethal amounts of diphtheria and tetanus toxin until the animals grew resistant, then showed that serum from those animals could both protect and cure others. Their December 1890 paper is the birth certificate of serum therapy; the first sick child was treated with diphtheria antitoxin about a year later. In 1901 von Behring received the very first Nobel Prize in Physiology or Medicine — awarded to him alone, though Kitasato, his co-author and the meticulous hand behind much of the tetanus work, was left off.

Serum therapy was the direct ancestor of every antitoxin and antivenom still made today, and of the "passive immunity" given as antibody injections. The deeper science it opened runs through blood typing, transplantation, allergy, and autoimmune disease, to the monoclonal antibodies that are now among medicine's most powerful drugs — and to the modern understanding of how vaccines, invented blind a century earlier, actually teach the body to defend itself.
