Sulfa drugs

The first family of antibacterial drugs that could be swallowed to cure infections anywhere in the body. It began with Prontosil, a red dye that Gerhard Domagk showed in 1935 could pull mice — and then people — back from lethal streptococcal infection. Sulfa drugs opened the antibacterial age a few years before penicillin reached patients.

The first antibacterial drug was a red textile dye — and the dye itself did nothing. Domagk's Prontosil only worked after the body tore it apart: a team at the Pasteur Institute showed in 1935 that a colorless fragment, sulfanilamide, was the actual germ-killer. Worse for its makers, that active molecule had been synthesized back in 1908 by a student, Paul Gelmo, and its patent had long since expired — so the real medicine could be owned by no one.

Sulfa drugs are synthetic sulfonamide compounds that stop bacteria from multiplying. They exploit a difference between microbe and host: many bacteria must build the vitamin folate for themselves from a small molecule (para-aminobenzoic acid), while humans simply eat their folate. Sulfanilamide is close enough in shape to that building block that the bacterium's machinery grabs it by mistake and jams — so the drug starves the germ without touching human cells. That selective harm is what makes it a medicine rather than a poison.

Before sulfa drugs, a systemic bacterial infection — childbed fever after birth, a spreading wound, some pneumonias and meningitis — often could only be watched. Sulfa drugs were the first pills that reliably cured such infections throughout the body, and they arrived in clinics around 1936, roughly five years before penicillin became a usable drug. They also proved a method: that you could hunt for medicines by systematically screening synthetic chemicals against disease, which reshaped how the pharmaceutical industry worked.

Sulfa drugs launched the era of designed antibacterial chemistry and spawned thousands of derivatives. The sulfonamide scaffold turned out to do more than fight infection: tinkering with it produced the first thiazide diuretics and some oral diabetes drugs. And by demonstrating, before penicillin, that infection could be cured by a manufactured chemical, sulfa drugs primed the world to seize on antibiotics the moment they arrived.
