Before anesthesia every operation was performed on a fully conscious patient, so surgeons were judged by raw speed — Robert Liston was famous for his speed, his fastest recorded leg amputation taking under half a minute, because every extra second was agony.

Anesthesia is the medically controlled loss of sensation or consciousness that lets surgery happen without pain. Ether, first demonstrated publicly in 1846, put patients into a reversible sleep from which they woke unharmed and remembering nothing. It made the operating table survivable in a way it had never been.

For all of history before it, surgery meant being cut while wide awake, held down and screaming; a surgeon's greatest asset was raw speed. Robert Liston was renowned for his speed — his fastest amputations took well under a minute — because every second was pure agony. Anesthesia removed the clock, and with it the ceiling on what surgery could attempt.

On October 16, 1846, the dentist William Morton administered ether vapor to a patient at Massachusetts General Hospital while the surgeon John Collins Warren removed a neck tumor painlessly. The room is still called the Ether Dome. Ether had in fact numbed surgery once before — the Georgia physician Crawford Long excised a tumor under it in 1842 — but Long did not publish, so it was the 1846 demonstration that carried anesthesia into practice. Chloroform and other agents soon followed, and the practice spread across the world within a few years.

Anesthesia made deep, careful, hours-long surgery possible — abdominal operations, and eventually organ transplants and open-heart surgery. Combined with antiseptics, it turned surgery from a last-resort butchery into a precise medical discipline. Modern operating theaters, obstetric pain relief, and intensive care all rest on it.
